    Community Health Needs Assessment Activities: 2021

    • The ACA requires all 501(c)(3) tax-exempt hospitals to conduct a Community Health Needs Assessment (CHNA) every three years, starting with the hospital's tax year beginning after March 23, 2012. The 2011 Schedule H included an optional section of questions on the CHNA process. This web tool includes responses for those hospitals voluntary reporting this information. The web tool will be updated to reflect changes in these questions on the 2012 and subsequent Schedule H forms.

      MEDICARE HAS UNDERPAID THE HOSPITAL FOR SERVICES PROVIDED TO THE COMMUNITY. WITH THE INCREASE IN SUPPLIES, PHARMACY PRICES AND LABOR WAGE HIKES THE MEDICARE RATE DOES NOT COVER THE COST OF THE EXCELLENT HEALTHCARE WE PROVIDE TO OUR COMMUNITY.

      PATIENT FINANCIAL ASSISTANCE POLICIES ARE AVAILABLE ON OUR WEBSITE IN PLAIN LANGUAGE SUMMARY FORM AND IN PAPER TO ALL PATIENTS. PATIENT FINANCIAL REPRESENTATIVES ARE AVAILABLE IN THE HOSPITAL AT THE TIME OF REGISTRATION AND DISCHARGE TO WORK WITH PATIENTS TO ENSURE THEY ARE AWARE OF FINANCIAL ASSISTANCE. ALL PATIENT STATEMENTS HAVE INFORMATION ABOUT FINANCIAL ASSISTANCE WRITTEN AT THE BOTTOM. WE HAVE CREATED A PATIENT RESOLUTION CENTER TO REACH OUT TO PATIENTS WITH OVERDUE BALANCES FOR ASSISTANCE IN APPLICATION FOR CHARITABLE SERVICES OR HARDSHIP DEFERMENT OF PAYMENT PLANS.

      WE SERVE THE LOWER VALLEY COMMUNITY WITH FRUITA COLORADO AS OUR HOME BASE. THIS IS A RURAL COMMUNITY LOCATED NEXT TO THE URBAN POPULATION OF GRAND JUNCTION COLORADO. THE COMMUNITY IS MADE UP OF FARMERS, RANCHERS, RETIRED FAMILIES, AND A BEDROOM COMMUNITY TO THE MANY RESIDENTS WHO WORK IN GRAND JUNCTION. OUR HOSPITAL IS THE FARTHEST WEST HOSPITAL IN THE AREA. WE ARE 19 MILES FROM THE UTAH BORDER RIGHT OFF INTERSTATE 7 WHICH RUNS FROM SALT LAKE CITY UTAH TO DENVER COLORADO.
