      "Facility , 1 - FISHERMENS HEALTH. For its FY2021 Community Health Needs Assessment (""CHNA""), Fishermen's Community Hospital (""FCH"") prioritized the needs and areas of opportunity based on input from the FCH community; analysis of local, State of Florida, and federal quantitative data; and focus groups. Baptist Health invited key stakeholders to one of three scheduled focus groups held via Zoom in March 2021. In addition, an online survey was emailed to all invitees who were unable to participate in the live Zoom sessions. A total of 27 community and internal stakeholders shared their perspective on the overall health of the FCH community. Internal stakeholders included various members of hospital leadership, representatives from case management, nursing, community relations, and physicians. External stakeholders included various community, non-profit organizations, local government leaders, and first responders. Focus group participants were asked to consider a compilation of data, which included statistical information focused on the FCH primary service area residents, and, when available, Monroe County, Florida and the U.S. and their personal/professional perspective during the qualitative question and answer portion of the focus group session. The focus group participants discussed topics such as behavioral health, access to care, and prevention and management of chronic diseases. Additionally, the groups discussed unhealthy behaviors that lead to chronic diseases, gaps and challenges in gaining access to comprehensive coordinated care for the uninsured, and improving the overall quality of life of the residents in the FCH service area."

      Facility , 1 - FISHERMENS HEALTH. For its FY2021 CHNA, Baptist Health conducted the assessment for Fishermen's Community Hospital. In order to ensure broad community input, Baptist Health utilized the existing CHNA Steering Committee composed of 25 representatives, including hospital representatives, with experience in assessing healthcare needs to organize Fishermen's Community Hospital CHNA approach. The report takes into account the latest government and public statistics, hospital data and feedback from the various focus groups and surveys.

Behavioral Health * Five Behavioral Health support groups were offered throughout the Baptist Health system and the system hosted a total of 729 participants. * Baptist Health is committed to improving the health and wellness of the community. The Community Health Department (""Community Health"") offers free in-person and virtual behavioral health education, focusing on topics such as stress management, mindfulness, meditation, and various mental health issues. These health programs and workshops are offered to the community at large and to smaller, restricted groups, in partnership with community organizations, or other media outlets, like Baptist Health Resource Blogs, Facebook and Instagram Live programs, Tik Tok videos, and articles for websites such as Healthline and newspapers. In FY2022, Community Health offered 268 behavioral health-focused classes with 9,172 community participants. * Baptist Health has been able to establish behavioral health partnerships to help support our communities through community benefit support. In FY2022, Baptist Health contributed $592,082 to support our partners. These organizations provide psychiatric assessments, free outpatient counseling, bereavement support groups, inpatient substance use treatment, and free/low-cost medications for Baptist Health's patients and our community's members. o Monroe County: 211 Miami-Dade and Monroe (Jewish Family Services), Autism Society of the Keys, Florida Keys Children's Shelter, and Island Dolphin Care o Miami-Dade County: 211 Miami-Dade and Monroe, Agape Network, Children's Bereavement Center, New Hope Corps, and Sofia's Hope. o Broward County: 211 Broward, Children's Bereavement Center, and Eagles Haven o Palm Beach County: Faulk Center for Counseling Although Fishermen's Community Hospital does not directly provide cash contributions to these organizations, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, Broward, and Palm Beach counties. * In FY2022, Baptist Health provided a total of 853 free behavioral health visits on Care-On-Demand to the community. Four of these services were provided to Monroe County residents. Although Fishermen's Community Hospital does not directly provide behavioral health support via tele-health, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, Broward, and Palm Beach counties. * Pastoral care services and Palliative Care for Baptist Health totaled $388,396 and $4.291 million, respectively. Although Fishermen's Community Hospital does not directly provide pastoral care services and palliative care, Baptist Health as a system provides support to directly impact Fishermen's Community Hospital and the other Baptist Health entities. * Baptist Health has implemented a depression-screening questionnaire in our primary care practices for all patients. Patients who screen positive will be further evaluated to inform treatment planning. Fishermen's Hospital has implemented the Columbia Suicide Severity Rating Scale (C-SSRS) in the hospital's emergency department. The C-SSRS is a measure used to identify and assess individuals at risk for suicide. Providers should use the scale as a measure of suicidal ideation, intent or plan, and past suicidal behavior. This scale is used to guide appropriate therapeutic intervention and to facilitate safety monitoring and planning. Baptist Health has implemented the C-SSRS at all of its entities across the system. Socioeconomic Challenges * Housing insecurities - Baptist Health, through its partnership with Habitat for Humanity, contributed to housing projects in Monroe, Miami-Dade, and Palm Beach counties. Specifically, contributions were made to help partially sponsor a home in Boynton Beach and five houses in Miami-Dade County. In addition, contributions were provided in order to complete a 4-Unit project, break ground on an additional 10 units in the Upper Keys, and complete 9 home units in the Middle Keys. In addition to employee volunteer hours, Baptist Health contributed $265,000. Although Fishermen's Community Hospital does not directly provide cash contributions to these organizations, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, and Palm Beach counties. * Baptist Health contributed a total of $226,098 to the United Way benefiting the communities of Monroe, Miami-Dade, Broward, and Palm Beach. The community plan fund for United Way disburses funds to directly support the community's residents through partnerships with local organizations, as well as United Way initiatives like the Center for Excellence and Early Education, Center for Financial Stability, Mission United, and more. Although Fishermen's Community Hospital does not directly provide cash contributions to these organizations, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, and Palm Beach counties. Availability of Primary Care and Prevention * Baptist Health Primary Care has two offices serving Monroe County. Although Fishermen's Community Hospital does not directly provide primary care to the community, Baptist Health Primary Care provides primary care services to directly impact the Fishermen's Community Hospital and the communities of Monroe, Miami-Dade, and Broward counties. * Baptist Health provides continuing medical education (CME) in support of its primary care providers and allied health professionals through its accredited program. Baptist Health continues to fund and support continuing medical education activities. It offered 511 conferences with 118,296 health professionals attending. In FY2022, 2,350 possible credits were offered. A total of 174,517 CME/CE credits were awarded as a result of participants completing more than one activity. In Monroe County, 244 healthcare providers received CME credits. Although Fishermen's Community Hospital does not provide CME directly to community physicians and health professionals, Baptist Health provides CME to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, Broward, and Palm Beach counties. * Baptist Health contributed $269,200 to support chronic disease management organizations in Monroe County. This included the American Cancer Society and Sabrina Cohen Foundation, which serve Monroe, Miami-Dade, Broward, and Palm Beach counties. Additionally, $25,000 was allocated to support the City of Marathon, National Fitness Campaign, for their fitness court's free community use. Although Fishermen's Community Hospital does not directly provide cash contributions to these organizations, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, and Palm Beach counties. * The Community Health Department offered free exercise classes, health screenings, and health education programs to the community at large. The Community Health Department hosted 2,441 exercise classes for a total of 81,918 exercise encounters and 881 educational programs - many focused on healthy eating and nutrition - to 25,921 participants. Of these programs, 483 biometric health screenings were performed, one exercise class was offered, and two chronic disease management programs were hosted at Fishermen's Community Hospital. In addition, five chronic disease management programs were offered with 60 total participants in the Fishermen's Community Hospital Service Area. * In collaboration with the Area Health Education Centers (AHEC), Fishermen's Community Hospital referred 11 patients to the Florida Department of Health's Tobacco Free Florida program."

Miami-Dade County: Good News Care Center, Homestead Community Health Center, Open Door Health Center, and South Miami Children's Clinic c. Palm Beach County: Caridad Center Baptist Health also made contributions to Chapman Partnership and Good Samaritan Clinic totaling $30,480 to support clinic operations and access to Care. In the Florida Keys, $75,000 was provided to Florida Keys Area Health Education Center to support the medical and dental program for school-age children. Although Fishermen's Community Hospital does not directly provide cash contributions to these organizations, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, and Palm Beach counties. * A total of 285 clinic patient encounters were conducted at a total cost to Fishermen's Community Hospital of $456,438. * Baptist Health provided $15.806 million in direct support to community physicians for medical care to treat uninsured patients and ensure access to providers in Monroe County and the areas of South Dade and the City of Boynton Beach. Although Fishermen's Community Hospital does not directly provide support to community physicians, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe and Miami-Dade counties. * Care On Demand (COD) is Baptist Health's telehealth platform and digital front door to access the system. Service is now available for multiple specialties to treat our patients' needs. The platform provided 21,213 virtual urgent care visits; of those visits, 11,455 were free of charge. In Miami-Dade County, 15,010 virtual urgent care visits were provided by COD, of which 8,263 were free. In addition to urgent care visits, Baptist Health offered specialty visits on the COD platform, including: 63 Diet and Nutrition and 4 Lactation specialty visits. Although Fishermen's Community Hospital does not directly provide telehealth support via COD, Baptist Health as a system provides support to directly impact the Fishermen's Community Hospital community and the communities of Monroe, Miami-Dade, Broward, and Palm Beach counties.       During part of the tax year, Baptist Health's financial assistance policy did not reflect a complete list of in-network and out-of-network providers covered, as well as all the BHSF wholly-owned Hospitals and affiliates. BHSF updated the provider list on September 16, 2022 and posted the updated provider list on BHSF's website. BHSF has also implemented procedures to ensure compliance with this requirement in the future.       Schedule H, Part V, Section B, Line 22 PROSPECTIVE MEDICARE METHOD
      During FY2022, BHSF did not specify the method used by the facilities to determine the amounts billed to individuals who have insurance covering emergency or other necessary care (AGB). This was discovered during FY2022 during a 501(r) compliance review (based on the final regulations) performed by BHSF and its outside tax consultants. BHSF did complete an analysis in which they believed they were complying with the 501(r) regulations as there was no charge to charity patients. Additionally, BHSF asserted at that time that no individual that qualified for financial assistance under the FAP was charged more than AGB. As suggested by the consultants, BHSF adopted the prospective Medicare method to determine AGB. Under this method, AGB is calculated by using the billing and coding process BHSF would use if the FAP-eligible individual were a Medicare fee-for-service beneficiary and setting AGB for the care at the amount BHSF determines would be the total amount Medicare would allow for the care. BHSF does not bill or expect payment of gross charges from individuals who qualify for financial assistance under this policy. In addition, BHSF has implemented a procedure to ensure compliance through various guidelines and communications to complete the AGB calculation and update the FAP as required annually.

However, BHSF in certain instances enters into payment agreements with patients that allow payments in excess of one year. For those cases, the financing component is deemed not significant to the contract. Generally, patients who are covered by third-party payors are responsible for related deductibles and coinsurance, which vary in amount. BHSF also provides services to uninsured patients, and offers those uninsured patients a discount, either by policy or law, from standard charges.       "Prior to Hurricane Irma's devastation on September 10, 2017, Fishermens Health, Inc. was an all-purpose, critical access hospital offering a 24/7 emergency room, inpatient and outpatient surgery, diagnostic imaging, lab testing, and physical and occupational therapy. Hurricane Irma struck just southwest of Marathon, causing substantial damage to surrounding communities. On June 7, 2021, a new innovative 37,330-square-foot facility was completed and opened to the public. The hospital includes 22 beds, including two intensive care beds, and is designed to withstand Category 5 hurricane wind speeds. The facility provides the community with a 24/7 emergency care with on-site helipad, diagnostic imaging, inpatient care, intensive care unit, telestroke services, outpatient laboratory and rehabilitation. Baptist Health made this investment in recognition of the significant community need, including providing care to those patients who are unable to afford healthcare.
