HOWEVER, MEDICARE PAYMENTS REPRESENT A PROXY OF COST CALLED THE ""UPPER PAYMENT LIMIT."" IT HAS HISTORICALLY BEEN ASSUMED THAT UPPER PAYMENT LIMIT PAYMENTS DO NOT GENERATE A SHORTFALL.
